Home Sweet Home Salud
On-demand video consultations with licensed doctors for tourists who fall ill in Spain. Triage, consent, payment, consultation and medical documents run in one guided flow, in the patient's language.
Demo with fictional data.
The situation
A medical service for tourists ran on a patchwork of chat, forms, payment links and video calls, which limited how many patients the team could handle.
What changed
A guided flow takes a patient from triage to a consultation and a full set of documents in their own language. Doctors are assigned automatically, the paperwork generates itself, and hotels can cover their guests through a new B2B channel.
Built for a medical service that connects tourists in Spain with licensed doctors by video.
“Gonzalo turned a service we ran by hand over chat into a real platform. Patients reach a doctor faster, and the paperwork takes care of itself.”
Goals
- Replace a manual service run across chat, forms, payment links and video calls with one guided, auditable flow.
- Get a sick traveller in front of a doctor quickly, in their own language.
- Produce the clinical and billing paperwork automatically, including reports, prescriptions and invoices.
- Open a B2B channel where hotels and holiday rentals cover the consultation for their guests.
What it does
- A patient flow covering triage with geolocation, consent with a drawn signature, card payment, a waiting room or scheduled slot, the video consultation and a resolution page.
- Automatic doctor assignment after payment, based on real availability. Children go to paediatricians first, the load is balanced across doctors, and a case is reassigned if a doctor does not start in time.
- A doctor and operations dashboard with episodes, clinical notes, availability, partners, invoices and reports.
- A post-consultation package with a report in the patient's language, a prescription, an invoice, and nearby pharmacies and health centres.
- B2B coverage, where an accommodation's reservation code skips payment and the partner is invoiced instead.
- A patient portal with reports, treatments, vitals and validated health questionnaires, with history charts.
- The patient interface is available in nine languages, with automatic detection.
AI and automation
- The platform is AI-agent ready. A secure MCP server lets AI assistants query episodes, doctors and invoices with the same role-based permissions as the dashboard.
- The episode lifecycle is a state machine that drives the rest automatically, from doctor assignment and notifications to documents and billing.
- Scheduled jobs send appointment reminders, cancel unpaid episodes and detect doctor no-shows.
- Email, WhatsApp messages to doctors and calendar invites go through a queued outbox with retries.
- The product was built with an AI app builder in plan mode, with automated security scans that caught and fixed an over-broad realtime policy before launch.
Engineering notes
- Postgres row-level security (around a hundred policies) separates patients, doctors and operations staff, and doctors can only subscribe to their own cases in realtime.
- Anonymous patient sessions use per-episode access tokens, so tourists never have to create an account.
- Invoices use numbered series by year, with a separate path for corrective invoices and a switch between test and live payments.
- A single shared PDF builder produces the consultation report, both before signing and as the fallback.
